To make sure you receive the current package price with all available discounts, use the current submission forms for the lab location where you are sending your samples.
Paperwork Reduction Discount - 10%
IF you have fully and legibly completed the Submission Form, you are submitting full payment* with the samples, and the samples are shipped
so they do not arrive on a Saturday or Sunday (the lab is closed on weekends), multiply the total due by .1 (10%). You may deduct this from the total due. The result is the amount you must submit with your samples. Otherwise we will invoice you for the amount due.
*You may pay with a credit card. Simply provide the card number,
type, name and address as it appears on the card, and the expiration date.
We accept MasterCard, VISA and American Express. Providing complete, current
information will be accepted as payment with your submission.

Useful information
how to calculate fees
Check the boxes for the assays you wish us to do. If sending more than one
sample be certain to mark the samples clearly so we can tell which is which.
If you want different assays done on some samples than on others, use a separate
Submission Form for each group of assays desired (i.e. all samples on a Submission
Form should have the same assays requested.)
If more than one box is checked for samples, add up the fees for all boxes checked to determine the cost per sample. Note that some sets of assays are specially discounted.
Multiply the number of samples times the cost per sample.
This is your total due.
Do this for each Submission Form if you are submitting more than 1 set of samples. Add the Totals Due together.
Shipping Samples from Overseas
Be sure to include a complete payment:
1. Check or Money Order in U.S. Currency or
2. Your currency is fine with a $25 processing fee or
3. Complete Credit Card info: Name, Type, Expiration, Billing Address, Signature.
