Sample Submission Checklist
  1. Print the appropriate Submission Form for the kind of sample(s) you want to send.
    Make multiple copies if necessary.
  2. Review the form.
  3. Review the appropriate “How to take a sample” guidelines.
  4. Take the sample(s).
  5. Complete the Submission Form(s).
  6. Calculate the assay fees.
  7. Include payment (or credit card information) to qualify for the paperwork reduction discount, if desired.
  8. Carefully pack the samples in a shipping container. Refer to ‘Shipping Considerations” at the end of the “How to take a sample” guidelines for details.
  9. Tape the completed Submission Form(s) (with your payment, if desired) in an envelope to the outside of the container.
  10. Send to Soil Foodweb Inc. by overnight mail if activities are desired. You may send second day mail if activities are not required. Ship so that the samples do not arrive on Saturday or Sunday. (We cannot accept samples on Saturday or Sunday – the lab is closed.)
